Promedica24 is currently looking for experienced and compassionate Self-employed Care Assessors to join our team on an ad hoc basis. This is not a full-time role,
but an excellent opportunity for those looking to support our growing
service as and when new clients require care assessments.

What we are looking for:
- Full UK driving license and access to your own vehicle is a must.
- At least 2 years experience in a similar or Senior Carer role.
- A minimum Level 3 in Health and Social Care Diploma (NVQ Level 3).
- Experience and understanding of electronic care planning platforms.
- Strong interpersonal and communication skills.
As a Care Assessor you’ll be responsible for:
Conducting Assessments:
- Meeting with potential new clients and their families to understand their needs and preferences.
- Identifying the type of care required.
- Assessing the client's physical, mental, and emotional needs.
- Completing risk assessments to ensure a safe environment.
Developing and Adapting Care Plans:
- Creating person-centered care plans that outline the client's needs and how they will be met.
- Including information such as personal history, medical history, allergies, and personal preferences in the care plan.
Pay:
- £130 per completed assessment
- Fuel £0.45 per mile
Location:
Various client locations – flexibility to travel is essential.
